I do not accept private health insurance; however I will provide an itemized receipt with my procedure codes for you to submit to your insurance company for reimbursement. If you are paying for massage with a flex-spending card, I will provide a receipt.

 

I will bill for auto accident claims and worker’s compensation claims. Please bring a doctor’s referral for massage that includes the referring physician's diagnosis codes and NPI number, as well as your claim number to your first visit.
